lombaire isol\u00e9e (ILEX) sur la douleur, le handicap et la fonctionnalit\u00e9 physique chez les patients atteints de lombalgie chronique non sp\u00e9cifique.<\/p>\n<p>Huit essais randomis\u00e9s ont \u00e9t\u00e9 inclus, impliquant 381 participants. Les r\u00e9sultats indiquent une r\u00e9duction significative de la douleur chez les patients pratiquant l\u2019ILEX par rapport aux groupes t\u00e9moins ne recevant aucun traitement (ES = -0.633, p = 0.004). Cependant, son impact sur le handicap (ES = -0.292, p = 0.190) et la force isom\u00e9trique (ES = 0.967, p = 0.150) reste incertain.<\/p>\n<p>L\u2019ILEX cible sp\u00e9cifiquement les muscles extenseurs du dos en restreignant le mouvement du bassin, ce qui am\u00e9liore la stabilit\u00e9 lombaire et r\u00e9duit la charge sur les structures vert\u00e9brales. Cette \u00e9tude propose une <span class=\"notion-enable-hover\" data-token-index=\"1\">nanoplatforme hybride<\/span>, Fe\u2083O\u2084@ZIF\u20108-RVC (<span class=\"notion-enable-hover\" data-token-index=\"3\">FZR<\/span>), combinant des nanoparticules de magn\u00e9tite (Fe\u2083O\u2084) et un cadre organom\u00e9tallique ZIF-8 pour un <span class=\"notion-enable-hover\" data-token-index=\"5\">traitement cibl\u00e9 et contr\u00f4l\u00e9.<\/span><\/p>\n<p>Le syst\u00e8me fonctionne selon deux principes :<\/p>\n<ol>\n<li><b><span class=\"notion-enable-hover\" data-token-index=\"7\">Ciblage magn\u00e9tique<\/span> :<\/b> Les particules Fe\u2083O\u2084 permettent une accumulation pr\u00e9cise du traitement au niveau de la zone affect\u00e9e.<\/li>\n<li><b><span class=\"notion-enable-hover\" data-token-index=\"9\">Lib\u00e9ration pH-sensible<\/span> :<\/b> Le ZIF-8 assure une lib\u00e9ration progressive des agents th\u00e9rapeutiques en r\u00e9ponse aux microenvironnements acides, typiques des tissus enflamm\u00e9s.<\/li>\n<\/ol>\n<p>Des tests sur mod\u00e8les animaux ont montr\u00e9 une <span class=\"notion-enable-hover\" data-token-index=\"11\">r\u00e9duction significative de la douleur<\/span> et une am\u00e9lioration des fonctions nerveuses, sugg\u00e9rant que cette plateforme pourrait r\u00e9volutionner la prise en charge des douleurs lombaires chroniques <span class=\"notion-enable-hover\" data-token-index=\"13\">sans intervention invasive<\/span>.<!-- notionvc: est un probl\u00e8me de sant\u00e9 majeur, souvent associ\u00e9 \u00e0 des modifications structurelles des muscles paraspinaux. Cette \u00e9tude vise \u00e0 analyser l&rsquo;asym\u00e9trie musculaire lombaire et son lien avec la cLBP en fonction du sexe. Un total de 250 participants (133 avec cLBP et 117 sans douleur lombaire) ont \u00e9t\u00e9 \u00e9valu\u00e9s via IRM pour mesurer la surface transversale (CSA), la surface transversale fonctionnelle (FCSA) et l\u2019infiltration graisseuse (FI) des muscles psoas major (PM), quadratus lumborum (QL), \u00e9recteurs du rachis (ES) et multifidus (MF).<\/p>\n<p>Les r\u00e9sultats montrent des asym\u00e9tries musculaires significatives chez les patients atteints de cLBP, notamment au niveau du QL chez les hommes et des ES chez les femmes. L&rsquo;infiltration graisseuse asym\u00e9trique est particuli\u00e8rement marqu\u00e9e chez les patients cLBP, corr\u00e9l\u00e9e positivement avec la dur\u00e9e et l\u2019intensit\u00e9 de la douleur (p &lt; 0.05). Chez les femmes, une plus grande asym\u00e9trie du muscle \u00e9recteur du rachis \u00e0 L5\/S1 et du psoas major \u00e0 L4\/5 a \u00e9t\u00e9 observ\u00e9e, tandis que chez les hommes, les diff\u00e9rences \u00e9taient plus marqu\u00e9es au niveau du psoas major \u00e0 L1\/2 et L4\/5.<\/p>\n<p>Ces r\u00e9sultats sugg\u00e8rent que l\u2019asym\u00e9trie musculaire et la d\u00e9g\u00e9n\u00e9rescence graisseuse des muscles lombaires jouent un r\u00f4le cl\u00e9 dans la pathog\u00e9nie de la cLBP, avec des diff\u00e9rences significatives entre les sexes. Les patients ont re\u00e7u sept s\u00e9ances de physioth\u00e9rapie sur six semaines, suivies d\u2019une s\u00e9ance de rappel \u00e0 11 semaines. Trois mesures de la douleur ont \u00e9t\u00e9 \u00e9valu\u00e9es : le seuil de douleur \u00e0 la pression (PPT), la sommation temporelle de la douleur (TSP) et l&rsquo;hypoalg\u00e9sie induite par l&rsquo;exercice (EIH).<\/p>\n<p>Les r\u00e9sultats montrent que ni la PiP ni l\u2019UP n\u2019ont eu un impact sup\u00e9rieur sur la modulation de la douleur. Cependant, une augmentation du PPT a \u00e9t\u00e9 observ\u00e9e dans les deux groupes, indiquant une r\u00e9duction globale de la sensibilit\u00e9 \u00e0 la douleur. En revanche, aucun effet significatif n&rsquo;a \u00e9t\u00e9 observ\u00e9 pour la TSP et l\u2019EIH.<\/p>\n<p>En analysant la relation entre les changements de la sensibilit\u00e9 \u00e0 la douleur et les r\u00e9sultats cliniques, les auteurs ont constat\u00e9 que l&rsquo;augmentation du PPT et la diminution de la TSP \u00e9taient associ\u00e9es \u00e0 une am\u00e9lioration de la fonction physique. Toutefois, aucun lien significatif n\u2019a \u00e9t\u00e9 \u00e9tabli entre ces changements et la r\u00e9duction de l\u2019intensit\u00e9 de la douleur ou des sympt\u00f4mes de sensibilisation centrale.<\/p>\n<p>Ces r\u00e9sultats sugg\u00e8rent que bien que la PiP int\u00e8gre une approche cognitive et \u00e9ducative pour influencer la perception de la douleur, son efficacit\u00e9 sur la sensibilit\u00e9 \u00e0 la douleur n&rsquo;est pas sup\u00e9rieure \u00e0 celle de la physioth\u00e9rapie classique.
